Brilliant, and creative, and original. My fav. lines were:
I will always be with you,
and if you sail away,
I will follow you.
I read those and it really struck a chord. Well done on relaying your feelings and emotions. You convinced your audience how this person makes you feel. and how you relayed how you promise to always love him or her. I would only suggest that you add things that this person does for you, and what you do for him and his or her attributes. Great job!

Yes a cento, is where you take lines from another artist, be it a poet, songwriter or something, and make another piece of work out of them, in this case I used Phil Collins. I have a collection of them...
