I imagine that if I were the subject of this poem, I would be quite gratified. It appears that the there is someone very important to you, someone who has given you reason to live.
Personally, I'm not a huge fan of the pink scheme you've got going on here, but it does help match the relatively saccharine theme portrayed in the poem.
Overall, very kind hearted and good.
