Mothers in Heaven.
Mothers on Earth.
Mothers not of blood, but of heart.
Who held the hand.
Who cried with them.
Who accepted them for who they were.

It's such a lovely sentiment. I had (and still have) a very good mom so I know how important they can be in our lives. The better the mother, the better the child, I've no reason to assume that isn't true. Nice sentiment here!

I think this is a wonderful tribute, but I feel it would be better without the repetition. Still, that's just my opinion. It's a good poem.
