A form of my own design. An unrhymed quatrain with the first stanza serving as refrain lines in each subsequent stanza. The first line of stanza one is repeated as the second line in stanza two, the second line is the third in the third stanza, the third line is the fourth line in the fourth stanza and the fourth line is the first in the fifth and final stanza. So that you have:
A,B,C,D
E,A,F,G
H,I,B,J
K,L,M,C
D,N,O,P
