You will be stealing lines from your own poetry.
The last line of the latest poem you have written will be the TITLE of your patchwork poem.
The first two lines of your second most recent poem will be the first lines of this new poem you are assembling.
Go to the next poem in your collect and take the first three lines.
Now go to the forth latest poem in your collection and take the first four lines.
The last three lines of your poem will come from the next poem in your collection. BUT these lines will be the LAST three lines of that poem.
Now you will have a 12-line poem.
NOTE: It's OK to skip a poem while you are stealing lines if you have been writing haiku and don't have enough lines in a particular poem.

NO erotica or adult themed lines or profanity
Your poems will be brilliant, trust me on that.
