This is my first contest...so pretty much anything goes...
a couple options..

I want a moment in time. A moment in your life that stands out...whether it be a time you fell in love, had your heart broken, your mom died, about your best friend etc...
[2]
A poem written for or about me.
(please put your option in the author notes, so I know which one you picked
I want only your very best, I know you all can do it
I want to feel it, like I'm the one in your shoes, going through what exactly what you felt. So use lots of wonderful language. Swearing is alright as long as it fits, but please don't over do it
