If I were to publish a book, do you think I should put every poem I've ever wrote in it or just the ones I've wrote from the age of 16+? I'm now 27 and have been writing seriousely since I was 11.

It depends on the quality. If you were going through the vanity press route, it would be entirely up to you.
If going through the selective route, the publisher would probably want to see previous published works (in magazines, anthologies etc') and would then give you some kind of 'brief' which might limit your selection of poems by theme, rather than age.
Good luck.

opinion
I have seriously thought about publishing a book also..in my own opinion, I have decided to just put the poems i think are appropriate for other people to read. So I really think it depends, some people may decide to write a book on all the poems they have written because thats their life work, and it took time and feelings and they want the world to know whats inside thier hearts and minds...but my advice to you would be to look over all your poems before you decide to put all of them into a book...I hope this helps you decide.

If you are going to publish this book , I would definatly have someone non biased to help you decide what goes in it or not. For one some of our favorite writes seem crap to others, If you are doing this for yourself and wish not to sale than use them all.

I self published
one book. I had, at the time, around 500 pages and used about eighty that I thought were good enough, {including around 10% previously published work.
For my second, the cut list will be larger...
